That’s what I would be looking at. I literally beat the s**t out of mine for several years with one tons and 42’s and never had a break like that. An entire aluminum adapter plate cracking in half would have me thinking about how it’s all mounted. I would have no issues throwing used stuff back in there, you have something else going on to cause all that damage.
Two t-case mounts. That's how it was when I bought it and when I looked around online maybe five years ago to verify that was correct, it seemed that was.Was there one or two tcase mounts?

I know a guy that has an R151 with an fj60 split case on it if this could help. Tough tranny and tcase.
